Roof Condition Certification: If your home is over 25 years old with a shingle roof, over 50 years old with a tile or metal roof, or if your mobile home is over 25 years old, you will need this form to obtain windstorm insurance. A licensed individual must fill out this form.
4-Point Inspection: Use this form to prove the roof, plumbing, heating/AC and electric have been updated. A licensed individual must fill out this form.
